Coaches:

1. Please fill out Game Card with all players on the roster. Record player first and last names in the numerical jersey order.

2. Please record both first and last names of Coach and Ast. Coach.

3. Record Region (181) Div. (example U10G) Team number.

4. Record Team Name

5. Record Team Colors

Referees:

1. Assure that First and last names are on game card, both players and coaches.

2. Assure that Team Id. number and name is recorded

3. Record date of match, Record Scheduled Match Time, & Field Number

4. Record Half time score at Half time in favor of Div# and team # (example: U12G12) (not yellow team or blue team)

5. At match completion record final score, winning Full team number example: (U12G13)

6. Record full Losing team number, example: U12G14 (not red team or team 13)

7. Print clearly (do not use cursive) the names of referee and both Ast. Referees.

8. If you want to get referee credit points please clearly write the team number to be credited. Example U14B14
